Dear Members of the

Health-care Community:

The Kresge Foundation is

requesting proposals for its Safety-net Enhancement Initiative, a new grantmaking

opportunity that seeks to foster new models of health-care delivery among local

and regional providers in order to reduce health disparities and improve the

health outcomes of adults and children living in underserved communities.

The Safety-net Enhancement

Initiative comprises two parts: a nine-month program planning and design phase;

and a three-year demonstration phase. During the first phase, planning grants

of up to $75,000 will be awarded to a maximum of 15 applicants. Responses to

the foundation’s request for proposals are due on November 16, 2009.

In the second phase, seven

to 10 of the phase-one grant recipients will be selected competitively to

receive three-year grants of up to $750,000 each to fund their proposed

demonstration projects. At the conclusion of the three years, the grant-funded

models and approaches will be evaluated, and the results will be publicized.
