I would love to have the panni first, but no way could I deal with this open incision. DO any other doctors do the panni pre-DS and close it up? Honey, he DOES close the incision in most cases! Mine elected to come apart-- excess fluids from too many fat cells is the likely culprit-- my hysterectomy incision was the same way-- we MO folks have a high incidence of wound dehiscence (coming un stuck). Dr. A also uses " retention " sutures in cases that are very likely to spring apart so you get a series of little openings to pack, not one huge thing. Once I came unstuck and we decided to use the VAC for additional closure, I escaped getting the retention sutures. Don't know what happened to Tom's-- his nasty infection may have had something to do with it. Plus, what invites infection is being MO and having skin folds that harbor germs, yeasts and fungus-- we are walking targets. ( actually, more like walking petri dishes.) I don't know the stats, but incision infections is far more likely in the MO than in the " normie " population. I don't know of any other doctors doing this pre-op panni-- they may be out there. Dr. Anthone specializes in very high risk super MO and high BMI patients (Tom was 69 and I was 71)-- he does this to REDUCE our risks. He has even invented a contraption to act as a giant retractor and hold up the abdominal fat and skin that constitutes the apron so his residents don't get hernias assisting in the surgery. Please remember that Tom's situation (and mine, for that matter) are NOT typical, even in the world of the Super MO! There are plenty of Dr. A's panni patients that have no complications. Tom and I are just hit the jack pot in surgical complications -- being a statistic is so much fun!
