Lucas' right foot now stays in the shoe (for the DBB) and doesn't budge at all. But now his left foot keeps slipping! I get it as tight as I can, and then I lace up the shoe even tighter. I've taken the tongue out of the shoe- he really liked that and was more receptive to having the brace put on, but he's still slipping. Any advice? I thought I saw someone mention they thought it was a sign of relapse... Is this right? I've also got him in socks with little rubber pads on the bottom to help prevent the slipping, but it still does. Little stinker! I do have an appointment for him with his doc so they can check his feet and the brace, but in the mean time I want him to still be in it. Thanks! Guest guest Posted December 19, 2005 Report Share Posted December 19, 2005 Jenn, I saw from your post on the yahoo clubfoot board that your son recently had some reconstructive surgery including the placement of pins. Did he just recently go into the brace? Do you think there's still some swelling that might be causing poor fit issues with the shoes? How do they have his brace set up? Is it similar to what they use for the Ponseti method with the open-toed shoes set at shoulder width apart with 70° abduction (outward rotation) and 10- 15° dorsiflexion? How do his feet look? Do they look completely corrected? Since you're not using the Ponseti method, you'll have to take the specific information/advice given here about the brace and adapt it for your son's particular situation. The settings used for the Ponseti method might not be comfortable for your son (especially the 70° outward rotation). Guest guest Posted December 19, 2005 Report Share Posted December 19, 2005 Thanks! I didn't even think that the advice would have to be tweaked a bit. His feet look great- no more swelling. The surgery was on the right foot, so I was thinking that maybe it was atypical and that's why it needed the surgery. But it's his left foot that was corrected with the serial casting that keeps slipping. I think it has relapsed some. I haven't even tried to get him back in it, because I don't want to force his feet a way that it doesn't want to go at this time. Does that sound right?? His brace is set up at 70 degrees, but not the 10 degree dorsiflexion. I asked the orthotist about the additional dorsiflexion and he said that's what they " used to do " and that it's not widely used today. Whatever..... from everything I've researched it is. But, I also don't think Lucas' feet can dorsiflex that much. He's been corrected to right at a 90 degree angle there, so I don't think he'd tolerate the extra on the brace. I do know that Lucas did fine with the brace until his foot slipped, then he cried (screamed). It took forever to get it to not slip....but now I think he could be kicking out of the left shoe because he lost some correction. I'd hate for him to be in casts (again!) for Christmas since he's getting some climbing and riding stuff, but I want his feet corrected so we can move on with the brace! Anyway, his appt is tomorrow with the orthopedist. He said to bring the brace so he could check it out also. I will let everyone know how tomorrow goes! Guest guest Posted December 19, 2005 Report Share Posted December 19, 2005 Jenn, Maybe that foot isn't corrected all the way and that's why it's coming out of the brace. If it was only corrected to neutral and not any amount of dorsiflexion, then maybe it was never completely corrected (by Ponseti standards) to begin with. Have you thought about getting a second opinion? If you tell us what region you're in, maybe we can recommend a doctor near you. It seems odd to me that your son's brace would be set up at 70° abduction if your doctor isn't using the Ponseti method. This could be why his foot won't stay in either. If his foot was never casted out at 70°, then it's not going to want to sit at that angle now...and it wouldn't be comfortable either. I understand not wanting to go back into casts since he's getting to be so mobile- but you'll want to get that foot optimally corrected and a cast now would probably be better tolerated than a series 3-4 months from now! The older they get, the harder the feet are to correct, and the less " adaptable " they are too. Let us know what the doctor tells you!
