Diet and family history questions

I'm in the processes of gathering all my documentation to send in

after my consult in October and I have a few questions about what I

should write.

When I write up a diet history do I just include dates and weight info

or do I include personal info as well or is that I separate letter.

I'm asking because I have only been on a program 3 times and a couple

attempts on my own that only lasted a few days at most. I don't want

the insurance company to just deny me because I haven't tried enough

things. It takes such mental effort to just watch food and exercise

that most of the time I just don't bother. But I've looked into some

of the medicines that are used namely Thermolift and another I forgot

the name of but they contained Ephedrine which from what I've heard

really scares me also I've nursed two kids for over a year each so I

couldn't take alot of the stuff even if I wanted to. I want to write

WHY I haven't tried alot of things (my mantra being I want to be

heathy not thin at all costs since I have no co-mobids and I think I

would have been worse off trying some of the drugs) and I'm not sure

if I should write one letter descrbing my weight history including the

diet attempts or if that should be a separate document.

My second question is what is considered " family " for family history.

Somewhere I saw something about only parents and siblings but I'm not

sure if that's correct. Many of my grandparents siblings have had

diabetes but it hasn't come closer than that. ALso my grandmother

died of a heart attack and one of my cousins had his first one at 35

but my parents and siblings are all fine (except their obesity).
