As for his foot slipping out of the shoe, did you check the bar length? If it is starting to get a little small this can contribute to being able to pull his foot out. The bar should be shoulder width from approximately heel to heel of the shoes - there are many different ways of measuring this but the easiest way is to hold the shoes up to your child's back and see if his shoulders fit easily between the heels of the shoes. If it is getting close, it might be time to widen or get a bigger bar. Having the padding in the back of the heel should not matter. Actually if you have the most current model of the Markell shoes (which you should, they've been out for a couple years now) there is a deeper heel pocket and more padding in this area so it is not really necessary to have the cushion in there and should not cause blisters - although I would check his feet daily with any change in the shoes, just as you would with a new pair. My son has had this issue where the nut would come loose, quite loose in fact since we had stripped the teeth on the bar from overuse. But we never ever had it come all the way off. BTW, if the nut is coming loose all the time, this could be a sign that either the bar or the plate on the bottom of the shoe is getting stripped. Also, by time is big enough to purposely unscrew them, I think he would be old enough to be taught not to. My Sammy is 13 months and he does not mess with his shoes or the nuts at all.
