Guest guest Posted November 28, 2005 Report Share Posted November 28, 2005 dear amy after correction in casts it is necessary to maintain the foot in full outward rotation (external rotation) and some dorsiflexion ( i.e. the foot faces slightly upwards from ground level). this can be achieved inside a DB splint whcih has its bar bent with apex of bend facing downwards (just like a warm smile on a bay's lips ar a crescent moon). the downward bend will have the shoes sitting in a lifted position. this will hold the feet in dorsiflexion. also, the baby will learn to kick his foot inside the splint after an initial tussle with adjustment for 2 or 3 days. the act of kicking means the forefoot hits on the floor of the shoe and lifts up which is nothing but dorsiflexion. so the db splint actually is " manipulating the foot " just like ur doctor did in the last casts. it is a dynamic splint. this dynamism requires the downward bend. a straight splint may not do so good. finally dont forget to peep thru the hole in the back of the shoe to see and confirm that the hell is in close contact with the shoe sole. regards murali. > > Amy, > > My son has the DBB ( gold bar) and we have always had a bend in the > middle.
