Tight, tight, tight. As tight as you can get it and then some. Push with your thumb on the strap after you tighten it, shove the heel deep into the shoe and then re-tighen. Then lace. Then pull on the heel to see if it comes out. If it does, re-tighten again. keep that up till it doesn't come out anymore.
