At the moment we have a pavlik harness, but the straps are lengthened on the foot/leg to allow for the shoe and then tightened in a way as to dorsiflex her foot, when she has the markells on, at least on her one leg. Then the straps for the other leg and the ones that would go over the shoulder were cut off. I'll send a picture of the contraption and what a normal one looks like to the CFPics list. My dad used to be an engineer and he had come up with a contraption that was basically a metal plate the shoe screwed into that extended out on either side of the shoe and there were metal bars with threads that were attached and the bars extended up the side of the cast and we held them in place on the cast with medical sports wrap stuff, I just had to be careful to make sure the shoe was at the right angle. When I took it in to the ortho to see what he thought he actually was going to cast the metal bars to her current cast, but the way the thing was fashioned there was no way I could unscrew the bottom so that the shoe could come off, you had to take the whole thing off. On another note, I didn't realize the pm's flexed at the ankle. That might actually make a difference. You don't think they'd come up too high on her leg do you? I know one of the other docs in the office has ordered pm's for other kids so they are familiar with them. Is there a website about them? I'm going to look into it. I figure if we make it to the 10 week mark (when she has her first cast change) with no relapse then we've figured out something that works and I'll stick with it...Otherwise, I have no choice but to have them cast the foot. And you're right, muscle atrophy is very typical in her situation. Just part of the deal though. No way around it... Thanks so much for your help!!
