Help with periactin

Hi everyone. I have a question. Today my daughter Autumn went to

her GI and they have decided she is old enough to start her on

periactin to increase her appetite. She is almost 11 months old and

she is supposed to start off with 1/2 tsp. twice a day. My question

for you, what is the best times to take it? Does it matter when? I

thought I had read before that it does. Her doc said to take it

maybe in morning and after a nap in the afternoon, does that sound

correct? Wish us luck. She had her blood test results done again to

see if her 'malnourishment' protein levels had gone up, and they

hadn't, and her doc said if this doesn't work we will have to

consider a feeding tube either through the nose or g-tube. She is

not doing horribly, 25-25 1/2 inches(depending on who's measured) and

13 lbs 14 oz, but they are definitly concerned. I'm pretty impressed

with our Children's Hospital, they are taking a very active role in

her RSS and are staying on top of everything.

Also, is there a lot of luck with appetite stimulant? Does it seem

to work most of the time? Just curious....the doc said it works in

some and not in others so we are in trial.

Here is the implementation schedule that MAGIC publishes for

parents -- based on feedback from Dr. Harbison and our RSS/SGA

parents.

First, we do not recommend 3 times a day normally, because, well,

the med seems to stay in the system about 6 hours. So if you give

it 3 times a day, you are very likely " double dosing " for part of

that time. Most physicians will prescribe twice a day. The total

dose should still be the pharmaceutical recommended dose of .25-

..4mg/kg/day.

Next. Timing. About 30 minutes before breakfast and 30 minutes

before dinner.

Third. Implementation. If your child is still napping every day,

you may find you can start at 100% dose and be find. For older

toddlers and younger kids who are no longer napping, we strongly

recommend the following implementation schedule. This takes a month

to get to the full dose.

Week 1: 50% of DINNER dose each night.

Week 2: 100% of dinner dose each night.

Week 3: Add in 50% of breakfast dose each day, while continuing

with 100% of dinner dose.

Week 4: 100% of breakfast dose and 100% of dinner dose.

If at any point you have had any major crankiness, etc., you can

dial back the dose. Usually it doesn't happen with the dinner

dose. But if at 100% of breakfast, you can't cope with child's

fatigue/crankiness, try 75% and move up slowly.

Hope this helps. Also, watch for ANY signs of reflux, even silent

reflux. This medicine can be a lifesaver in terms of caloric

intake, but is notorious for exacerbating reflux. Usually Zantac

will cure that.
