Then I take that letter and get it signed by her doctor, and then bring the signed paper back to the WIC appointment. I wouldn't trust them too much on helping me or anyone else anymore. They even turned me in to the CPS one time, because of my FTT daughter, so I would be careful with them. Just go in, and ask for that form, I think it's called a formula request form, but I need to double check. They now now, that she really needs this Pediasure and don't even ask me anymore questions, and happily give me the vouchers for the 20 boxes (or 120 cans) a month. It's free, and why go through all the trouble with your insurance company, even if you are on medicaid like us, or a regular insurance company. That is about 20 boxes, or 5 cases. We do have to go back to the health department every 3 months instead of 6, but it is nice to know that we can get our Pediasure through the health department and with no questions asked from them.
