I am with Great West PPO (OneHealth)

Work in the State of Kansas, Live in Missouri

Approved after first request for pre-approval and then a request from

insurance company to my surgeon for more info on their surgical practice.

They wanted to make sure that the surgeons do more that just WLS. Insurance

company also requested history of dieting efforts. Approval actually only

took a matter of days once they got the additional information they wanted.

Total process took about 3 weeks.

My surgeon was Dr. Warden in Ocean Springs, MS

My BMI at the time of surgery was 52 and I was approximately 160 pounds over

the " ideal "

I was approved for surgery at the end of July 2001 and had surgery at the

end of August 2001.
