Please help: insurance precedent.

Hello to all.

I live in Charlottesville, Virginia (born and raised in L.A.; what a

change!) and am trying to get approved for lap D/S with Dr. Gagner.

I need any and all information that I can get about my insurance

company approving D/S and/or their allowing the use of a surgeon out

of state (but paying as though he were in-state).

I have Qual Choice insurance, which was just recently purchased by

Coventry Health Care, and combined with their other company, Southern

Health.
