It's usually more gradual, though there are faster moving forms of pulmonary fibrosis. The other thing that can come on very quickly is pulmonary hypertension. Some of the symptoms of which are similar to pulmonary fibrosis. This is why it's critical to have regular echocardiograms. Pulmonary hypertension is one of if not the most common complication of pulmonary fibrosis and untreated is deadly.
