I know how frustrating a situation like this can be! All I was trying to get across is that the more attention (bad or good) that she gets because of an accident, the more often it will occur. Four-year olds are extremely self-centered. The world revolves around them, no matter how many other things YOU need to accomplish during the day. So if you can concentrate the attention you give her on the good things she does, and give her as little attention as possible for the bad things (in this case the accidents), hopefully the situation will turn itself around.
