She needs IV fluids and dextrose to flush those ketones out and get her to tolerate her feedings again. You know the routine: 24 hrs of gut rest, then try 25% formula with the rest Pedialyte, then increase to 50% if she tolerates that for most of the day, then so on. Actually, you should start with just Pedialyte after the gut rest. Do NOT go home until she is tolerating at least 50/50, or more.
