It shouldn't be started before the age of 2, because under the age of 2, GH doesn't not rule growth, caloric intake does. Dr H believes that a child should attain their caloric catch-up BEFORE beginning GHT. For my g-son, , this happened by 2 years of age, but he is 100% g-tube fed. For oral feeders it could be much later. From what we could tell, Dr H felt that had attained his caloric catch-up when his length measurements seemed to plateau while his weight measurements kept rising. Does that make sense?
