There is a lot of " lore " to the effect that we absorb somewhere in

the neighborhood of 30% of the fats, 80 % of the protein, and 100% of

the SIMPLE carbs (maybe less of the complex carbs). Since fat has

more than twice the caloric density of protein or carbohydrates, (9

cal/g as opposed to 4 cal.g), the 30% nominal absorption of fats

makes them no worse for our weight control than protein or carbs.

But, I have seen different stories with quite a bit of variation.

(Add a Krispy Kreme or your favorite greasy treat to flush out the

digestive system, and maybe we absorb even less --HAH!)

So, the answer to your 800 calories/day (which is frightfully low for

a DS post-op, by the way!!) depends on the relative proportion of

carbs, proteins and fats. For example, 300 cal of protein, 300 cal

of fat, and 200 cal of carbs would yield about 240 + 100 + 200

calories, or about 540 calories in all.

Morning Sue,

I asked that exact question of Dr. Rabkin at his seminar because I

had seen so many percentages of malabsorption thrown about. His

answer was very interesting. He said, " NO ONE KNOWS " . No one knows

because no scientific study has ever been done. He said the evidence

of fat malabsorption is in the well, the pooping...so one can see

that. Also, the proof is in the pudding in the fact that when

caloric intake increases, there is no or not a substantial weight

gain. I asked him specifically that I had seen that there is 80% or

60% malabsorption of fats, a slightly higher absorption of carbos and

total absorption of sugars, and he was quite amused by this. Oh, by

the way, Dr. Rabkin is vehement that no sugar be ingested during the

window of opportunity.

Now I have scoured everything I can find, and I cannot find one study

that indicates rate of malabsorption. Also, we do know that the DS

is putting Diabetes into remission within weeks of the surgery,

before any weight loss could impact the disease. So, although there

are no studies explaining the mechanism, there is the proof of it

happening! How's that for no information???

Yes .....right from the horses mouth! He not only told us that at one

support group meeting but told both Mike and I the same thing during our pre

opt testing when we signed the papers to go ahead with it. We had been

discussing how eating would change by one year out.....

Thank you, Theresa! LOL. I have also found it amusing when people give

exact percentages of absorption, since I have never, ever read any study

to back up these statements. But as you said (sort of) the proof is in

the pooping. :)

Dr. Scopinaro did do some studies on his BPD patients and found that

there is an apparent maximum absorption rate.

Read that section. It's long and dry to read, but the text and the

tables illustrate clearly how this procedure works and works well, and
