Call for diet history letters from docs

Hi all, I need your help.

This may not even mean anything, but I have to try. My Kaiser doc will

write a letter for me about my recent 1200 calorie diet with

them. However, I she doesn't have the time to put it together, so i am

going to give her a draft to edit and sign.

Does anyone have any good letters from doctors stating a particular diet

they went on, and perhaps with supportive language that diets just haven't

worked for their client, and surgery is recommended??

Thank you very much. i will have more stuff later on this, you guys on

these lists are my lifeline.
