I know there are DS surgeons that don't charge a program fee. There is one in

Michigan (Grand Rapids, I think). I believe Dr. Hess also doesn't charge a

program fee, but I could be wrong. I am sure other people on the list can tell

you what surgeons don't charge the fee.

Dr. Elariny in VA does not charge a program fee, but I think that he

now asks for a $2,000 advance on insurance reimbursement--to help his

office with cash flow issues as insurance is often slow in

reimbursing.
