You can't wait until the clinic because most people attend the clinic on the Tuesday the week of their surgery, and you can't get an approval for a surgery date until you have your packet turned in, so get it it if you are ready for the clinic and surgery. As for the length of the patient history, I can't help you there,, I never used to go to the doctor before except for diet pills so my history was very bare.... just include everything you can. My husband's company did override and will cover " out of the network " . I handled the insurance issue first and then got my packet together and took it to clinic with me (which he read through and asked questions) and then appoved me for surgery....my date is Sept 11. So far I think I'm the only one scheduled that day. My thought was that I remember being told early on that I needed insurance approval as well, but when the office called and said you are approved let's schedule surgery, they never asked me anything about my insurance. I thought that was interesting. Who knows maybe it was just an oversight by new staff.
