I'm working on getting my packet together...making contacts and starting on the patient information form. About your previous medical history-how far back is Dr. Rutledge interested in, and for previous surgeries...do we include things like appendectomies/tonsillectomies don 30+ years ago? Also, when the packet is complete, should I bring with me to my clinic appointment, or mail it before my appointment so the Dr. will have time to read it before we meet? I've read the manual, the paper, and everything on the mgb site, but I believe my brain has just turned to mush over all the information to absorb...can anyone relate? Also, I work at a hospital and we are self-insured. I've already been told that this is not a covered procedure so of course there will be an appeal. From my understanding, my employer can override the denial, and the appeal process will take place at the hospital.
