Also, Dr. Rutledge has a list of patient email addresses of people who have said they would be willing to be contacts at: ~Didn't know if you wanted to have the 'e' in the word 'forthe', and where it looks like there should be a link to Dr.R's site where the email addys are, there isn't anything... I do have two questions: You mentioned you mistakenly went to Dr. R's office instead of directly to the clinic on the Tuesday before your surgery. At Dr. R's office they gave you your file and then you went off to clining. Does this mean everyone has to get their file from Dr. R's office before they go to clinic, or should Dr. R's assistants bring your file to clinic if you are scheduled? You also mentioned in the 12 step process about the 3 signed agreemements - on abstaining from alcohol and certain pain meds, keeping in contact, and gettting annual blood tests. I have also seen this on Dr. R's site. Do we make up our own agreement texts and sign them for inclusion in the packet, or is there a formal agreement form? Getting my packet ready - hopefully I can send it in by the end of the week after I seem my PCP for my pre-op exam. There was also a bad link on that page 3 times for the physical where I had typoed an extra letter onto the address so that didn't work and I found it looking for the one you mentioned!
