The DOC Band was approved by them for treatment, but the location, Cranial Tech, was " out of network. " Was the Doc Band itself approved by them for you? If so you will have a good chance of them granting you the waiver. If you don't know, call the insurance coordinator for Cranial Tech and/or your insurance company to find out. About getting the waiver: Call BCBS and ask to be transferred to the waiver department. Tell them you want to request a waiver to go to Cranial Technologies. If you had the DOC Band approved, that means they will have to find another location within 50 miles that also has the DOC Band, which they won't, so therefore your waiver should be granted. At least that's how it worked with us (I'm pretty sure at least). Oh, and keep calling the waiver department after you make the request, because they seem to be very lazy over there and won't get back to you unless you keep calling them.
