I followed the guide that was posted and they said that it wasn't

enough. I just changed it to go along with the paper you download. I

just repeated what was said in that. I hope it works!!!

Donna, I worry that your reply is taking so long! I certainly don't think I

patient letter from twilight zone

Barb just told me to redo the patient letter because I didn't cover

each topic. I redid and put the same headings as the list. I sent

certified mail and it did get there about 3 weeks ago so I am just

waiting to hear.

I'm ready and I'm self pay. I just want a reply of some kind

(obviously, I want the thumbs up reply - !)
