All 12 headings (which are outlined in the manual) must appear. I think it's a bit much, too. But if you want to dance, Dr. must lead!! Also, for anyone else who might have this insurer, I just phoned Empire BCBS (NY State) to check on my pre-determination process since the letter went in last week, and so far it's not in the computer, even. The rep said that pre-determinations take 4-6 weeks. She said that this surgery is considered " cosmetic " . Looks like my idea to give myself the best birthday present ever (10-22) isn't even close!!!! Oh well. It will happen. They haven't even begun to see the fight I will give them, lawyers and all. They have become a little more complicated since the patient load is so heavy. My letter was that long. Good luck on your quest and may God bless you is my prayer. I was a little scared when the first date available to me was a little under three weeks. Could I get everything ready? Travel, car, hotel, kid coverage? Sometimes delays are blessings in disguise. Hope you have success with your insurer. I too am in the process of an appeal. I decided to go ahead with the surgery and then fight. The only other person that had this surgery with Dr. R that has the same insurance I'm fighting with (Premier of Missouri) has been fighting since January. She is nearly 100 pounds lighter. I also am hoping to schuedule surgery in october. My patient letter is 13 pages long. It took FOREVER to write. I think Dr. R is just covering his butt, he is proving that we know all the risks of this surgery, and he has done proper teaching on aftercare etc etc. I am planning on sending my packet out on Monday or Tuesday. I sent mine late on a Tuesday night and got my insurance letter E-mailed as an attachment on Wed. afternoon. I think you should just verify that they received it. Good Luck! Also I am going to make my appointments for my physical, mammogram, etc. I'll call you around 11:00. Also, I'm going to get a job in a Nursing Home so I can make more money in case I have to pay cash. How much have you lost so far!!! Hang on in there! I called them myself and found out that gastric bypass surgery is the ONLY weight loss method they WILL pay for. This is BCBS-Federal. So I hope that Dr. R will accept this sort of answer from them. When I was doing the letter, I felt in some places that I was repeating myself, but I was following the guidelines of the sample letter and it turned out okay. Good luck. I haven't sent mine in yet, but I was worried that I used a 12 font and 1 1/2 " margins and he wouldn't accept it. I got 12 pages. I took Dr. R.'s proposed test and made sure those answers were included in the letter. I bolded the test answers. Answering the test questions helped make it a little longer.
