forms and such? I don't know what I am supposed to

Dear Kristi, Bring a copy of your online information form you first

sent in to Dr. R, or you will have to fill out another one at clinic.

Take a copy of your EKG and your Blood Lab results. I never showed

mine but sometimes you do. Take a letter of your approval from the

insurance along with your pre-certification number. They need that

number at the hospital when you are being admitted. I am sure

everything will go great for you next week.

I don't have a copy of my medical records, they were

faxed directly to Dr. R. Is that ok?!?!?!!?

Also, am I correct when I say we go to Dr. R's office

and then to clinic? Where is the clinic? I am

printing all my maps out and realized I have no idea

where I am going... HELP!!!

Hey ,

Aren't you so excited... actually I have no clue what to pack.. I

can't believe the date is almost here,, and whats up with this

HURRICANE????? I'm in South Florida and freaking out because I'm

afraid my flight will be cancelled.... we already agreed we are going

to drive if it's cancelled.. yuck..

I aked Barb. how much we need to bring and she said be prepared to

pay your co-payment.. so my copay is about 20% and I think I have to

pay the full $255.00 for the clinic.. so I will have my VISA warm and

ready... if you are paying cash for your co-pay I'd be prepared..I

called visa and asked for a limit increase in case I have to fork

over a few thousand..

Can't wait to meet you, I'm bringing my camear and laptop so we can

download the photos ASAP.

Dear Krist, Yes you go to Dr. R's office first and they give you a

map to the clinic which is real close. Don't worry they will have

your records in your file. I never needed mine anyway, but I did need

my precertification # for the hospital. Everything will come together

for you Kristi.
