I think they email it to you and then you print it off. You have to send it in to your Ins. co., Dr. R's office doesn't do it anymore, while you're waiting to see whether your insurance will approve the surgery you can be working on your packet. On contacts what I did was email them privately and ask if they would mind being a contact for me and if they said yes I emailed again privately a list of questions that I wanted to know about. When they emailed me back with the answers I printed them off and studied them. I hope this helps you!!
