Hi ,

I see that you finally have some testing scheduled. Congrats.

That is a major first step to finding out what is making you so

sick! I have SOD and just had my second sphincterotomy done in March

of 2004. The ERCP w/manometry is not a painful test ie: you are

given meds to basically knock you out of it. When you come to, that

is when you watch for any signs of pain, etc from the procedure

(such as post-pancreatitis). The manometry is to measure the

pressure in your bile duct and your pancreatic duct. When I first

had manometry done on my bile duct it was found to be fine with

normal pressure. My second one found high pressure in the bile duct

and scarring and stenosis too. My third ERCP w/manometry they found

high pressure in the pancreatic duct, did a sphincterotomy on the

pancreatic duct and enlarged the prior sphincterotomy and dialated

the sphincter of oddi too. I did get pancreatitis after the

procedure and had to spend a few days in the hospital, but they took

very good care of me. I also wanted to tell you that your pain

pattern sounds alot like mine. Before my last ERCP w/manometry, I

would wake up some nights with alot of pain. Now, after my last

ERCP, my pain pattern starts out dull in the morning and progresses

throughout the day. Usually after I eat lunch is when the pain

intensifies. Some days I feel very good. So far I have had a run

of 7 days in a row without pain. Then last sunday I woke up feeling

ill and the pain under my ribs started again. It got worse sunday

night, and continued monday, and today. Right now, I feel like the

pain is lifting. I can always tell when I am close to getting over

a " spell " because I start to get very hungry, which is a good sign

that things are calming down. Well, I hope some of this helps.

Gosh, I'm over 1000 messages behind. Anyway, I happened to catch your message

about alt and ast. These are liver enzymes. Basically, they are in the liver and

when the liver is under siege it releases these enzymes into the blood. May I

ask which particular liver function test you are referring to and how high it

was. As I have chronic Hepatitis C I have these tested periodically especially

the alt and ast. This link gives a very basic explanation. Hope it helps.

Well, I've just returned from the GI doc appointment. I'm going for

an MRCP next week here in town and then am scheduled (earliest

available) for ERCP with manometry at the Lahey Clinic in Boston.

My GI doc does do ERCPs but he says that just an ERCP won't show

what he suspects is happening -- Sphincter of Oddi dysfunction. A

blood test that my surgeon took on the 6th of March (!!) which was

reported to me as normal showed a high level on one of the liver

function tests which is what he thinks may be the first clue that it

is SOD. Now why the surgeon never reported that to me or anyone

else is another matter and to be honest I'm po'd about it. I didn't

care for him anyway, his bedside manner was atrocious and talk about

an ego!! Well, don't get me started about him. Anyway, GI doc

today sent me to the blood lab to look at AST and ALT levels....does

anyone know what they are? All he said with respect to the blood

work that was done on the 6th of March was that the highs in the

liver levels shows that there may be a blockage or stricture

somewhere and things aren't " flowing " as they should. Anyway, just

thought I'd let you guys know.

On another subject we are going to have to do a re-finance on our

house as it looks like I'll be out of work for awhile and the cash

out we take on the equity will have to serve as my paycheck for

awhile. I've gotten the paperwork completed in order to be

officially on the family leave medical act so will send that in with

my hubby tomorrow.

Kris: My goodness!! You've had 3 of these? And the first they

didn't see anything??? Oh god, I pray they see something on the

first shot....but yes, I do feel some sense of relief because I just

know in my heart of hearts that this is the problem. I'm guessing

he ordered up the MRCP first to basically rule out any structural

abnormalities? Like cysts, etc.? Thanks for explaining

your " pattern " as well.....that is EXACTLY the pattern for me and

EXACTLY where I feel the pain.....UNDER the ribs and in the dead

center of them (in-between kind of.) I get this pressure in my

ribcage like something is about to explode, and then the nausea and

the burning pain comes -- and comes and goes in a sort of wave-like

pattern. It is an every day occurrence now. I'm just grateful that

this particular doc is not strictly looking at the lipase and

amylase levels as indicators.......thank the good Lord above!!

Thanks Jill...the level that was up early on is SGPT. Don't know

what that stands for..do you know anything about it? I'll look it up

on the link you sent me.

I had the ERCP with manometry in Dec and it showed I had SOD. The doctor put a

stent in at that time. I do believe the doctor is right because after my ERCP I

ended up having ampullary edema. I'm not sure if that is correct but anyway me

liver enzymes were high. The doctor said that any time you have an ERCP there

is a risk of having a panc attack. I do believe that having the ERCP with

manometry will help the doctor give you a definite diagnosis. I think most

people who suffer from CP have SOD.

SGPT is the " old " name for the ALT. The AST was know as SGOT. For many years I

was tested for the SGOT and SGPT. When they changed them a number of years back

it was hard to get used to it. In fact, there are times that my doctor refers to

them the old way. Did the doctor tell you how high it was? Enzymes can elevate

for a variety of reasons. Yes, ductal obstruction is one, but also gallbladder

problems, various meds, alcohol, fatty liver, viral hepatitis, and I'm sure

there are others. Please note that level of elevation does NOT correlate with

the amount of damage.

Thanks Patty..I know what edema is but what ampullary edema? He was

checking ALT and AST levels yesterday in another blood test but

won't have those results until next week sometime - don't know why

that is but that is what the people in the lab told me. I looked up

AST and ALT levels and they are the exact thing (the ALT anyway) as

what the surgeon should have made mention of to me back in March.

They start showing up if there is " cell death " happening in the

liver.

Hi Jill: Yes I figured that out when I did a web search on them.

The blood work the surgeon ordered back in early March showed it as

SGPT. I have a copy of that report and it was 62 or something. The

surgeon never made mention of it to me....but this was after the GB

was removed. About a week after I guess. Yes, in my reading last

night I noted that someone with severe liver damage may have a low

reading and someone with a case of Hepatitis A (that resolves

usually leaving no liver damage) can have a very high number. So,

it has to be looked at " looking at the whole clinical picture. " Let

me know what you think ok?

Although 62 is outside the " normal " reference range it is not a particularly

high number. I'll be interested in the results of your most recent test (btw,

the turnaround time for these tests is only a day or 2 not a week). You

mentioned the 1st test was done shortly after surgery and certain anesthetics

can affect enzyme levels. Also remember that certain medications can also affect

them. I don't know what meds you are on but you might also want to check with

your doctor to see which ones, if any, can affect the liver.

I agree that you and your doctor need to look at the entire picture. Is this the

1st time that you know of that they've been elevated? If the one you just had

done is also elevated maybe a hepatitis panel might be in order. Hopefully, they

will be normal and just a result of the surgery.

Thanks Jill.....I called the doc regarding possible anemic symptoms

and the blood work was in today......I was told that it was normal.

So, the first one back in March was right after the GB surgery and I

guess I'll just assume that it was due to the surgery.
