Debbie,

Try this to get your patient letter info. completed. Go online, pull

up the page in your browser, keep your browser open but log out of

your internet provider, then finish the form offline, go back online

(meanwhile keeping the browser open and with your info in it) and

then hit the " SUBMIT " button.

If you are on AOL, I don't think the AOL browser will remain open

if you log out of AOL. But, what you can do is you can log into AOL,

open another browser like Netscape or Explorer, bring up the page in

the non-aol browser, log out of AOL and proceed as above. When you

get

the letter done in the non-AOL browser, log back into AOL and then

submit the form from the non-AOL browser. I have done that with AOL.
