Kari-

I would suggest:

1) print out all of the information that you want to present to your PCP

2) complete the online patient form

3) while waiting for your insurance form to come back from Dr R (which only

took a few days), take the information to your PCP so that she can start

preparing for a referral.

4) When you get the insurance form, make sure your PCP has contacted the

insurance company and then fax or send the insurance form thru.

That way the insurance has the referral before the insurance form. Mine is

currently hung up waiting for the contact from my PCP because I did it in the

wrong order!

Hope this helps...my insurance said they cannot do a thing until they hear

from my PCP (who happened to be on vacation until yesterday!)
