Herman, M.D. I tho't I had it on disc but evidently I messed up saving it. I do not know how to forward it from the scanner. Dum, Dumb. But, here is an 800 number that is at the top of the article (800)998-6374 and . I don't have an address of who and when this was sent to us. I'm so sorry, but I am so sick I can't check any further right now. Sorry, but it has taken me forever it seems to send this. Shall I copy it and send by snail mail.
