Pre-Op checklist

I can't believe I'm going to say this but I wish 's old site was

still up. She recently had a checklist on there of what to do and

take with you for surgery. I never printed it out and now I wish I

had. I believe one of the post-ops prepared it for her. Does anyone

out there have it?????
