As a matter of fact there is another Dave having the surgery in October along with his beautiful wife . I am not sure if Ken and Bob have had the surgery this week? I would find out from BCBS what they are requesting from Dr. R and then make sure Dr. R's office has received the request(it is easier to email your questions). Also follow up on Dr. R's response back to BCBS (if possible get the office to send you a copy of the info they send back to BCBS). If you can get dates these things are done. Good luck!
