After 30 yrs of being overweight, and 20 years of being morbid obese, I've

researched weight loss surgery.

I feel very confident in MGB, and Dr. Rutledge. If I change my mind you'll be

the first one on my list to email.

Questions about the Loop Procedure for Weight

Loss - Dr. R?

Does any other weight loss surgery other than the MGB cause bile

reflux? (If yes, please name it)

Did the Mason Loop gastric bypass result in bile reflux?

Does the Billroth II Loop gastric bypass result in bile reflux?

Isn't it true that the word " loop " in both the " Mason Loop " and

the " Billroth II Loop " gastric bypass refers to a length of bypassed

intestine in both procedures that is connected to the new stomach by

an anastomosis?

Isn't it also true that it is the loop of bypassed intestine, joined

by an anastomosis to the new stomach, in both the " Mason loop " and

the " Billroth II loop " that results in bile reflux into the new

stomach?

Isn't it also true that attaching the loop (anastomosis) " lower " on

the new stomach pouch does not preven bile reflux into the stomach

through the anastomosis in either a Mason Loop or Billroth II loop

bypass?

Isn't it also true that whether the stomach is cut horizontally or

vertically, the shape of the stomach does not prevent bile reflux

into the stomach through the anastomosis in either a Mason Loop or

Billroth II loop bypass?

To your knowledge, is any other bariatric surgeon in the world doing

the Billroth II loop or the Mason Loop in any gastric bypass

procedure for weight loss?

Bile reflux occurs normally in the stomach, it occurs after RNY but it

occurs somewhat more with the loop type surgery like the Mini-Gastric Bypass

than RNY...

But the RNY has many more serious and severe complications. 30% have the

Roux limb syndrome with severe abdominal and persistant nausea, the weight

loss ususally fades at 3-5 years and revising an RNY is a complicated,

dangerous and difficult procedure. Bowel obstruction, leak, hernia, wound

infection, ulcers, pulmonary embolus and death are all much more common

after RNY.

If you feel that a little occasional bile reflux is a big deal don't have

the Mini-Gastric Bypass.

Have and RNY or a VBG or a Band or a BPD/DS...

But first talk to those patients and compare the results.

and what will you do when your RNY fails in 3-5 years?

Yes it was placed up near the esophagus and was abandonded.

Sometimes a little, usually early after surgery.

No one has had a significant long term problem from bile reflux.

We have had some ulcers in our series but they were from acid and were cured

with anti-acid drugs.

But our ulcer rate is lower than that reported by others after RNY.

Well it is not the stomach that caused the problem in the Mason Loop it was

the esophagus.

Loop type surgery is done every day by general surgeons in America (about

16,000 times per year)

Why not get excited about all of those surgeons doing this same loop

surgery?

As a test call your local general surgeona dn tellhim/her that your mom or

dad is having ulcer surgery and the surgeon is going to do an " Antrectomy

and Billroth II Loop gastrojejunostomy " .

See what he/she say.

They will say " Fine, sounds good to me. That is what I would do. "

Then you could start getting all wound up about the loop vs. RNY and see

what the surgeon says.

Nope you are confused.

The loop type Billroth II surgery is done everyday as a standard surgical

procedure for trauma, stomach cancer and ulcer disease of the stomach.

You are confused.

This is just my opinon, it is not fact, it just my observation.

It appears to me that the questions that are surfacing about the

safety of this procedure are not really about answers. It appears

that they are in an attempt to scare " pre-ops " . But this is not

working, least of all for me. I have been obese nearly my entire

life. I often joke and say " I was born weighing 6 pounds 7 ounces

and I've been trying to get back there ever since! "

While I know 100% of the people cannot be pleased and may have less

than stellar results, the truth is that the risks associated with ANY

of the gastric procedures are less than the health risks of remaining

morbidly obese. This we can agree on.

I am so grateful to ALL of the surgeons and their techniques. Like

most of the folks on this list, I chose Dr. R, but I could have very

easily opted for RNY. The decision was mine and your decision should

be yours to make.

I respect that all of the questions should be asked and answered

BEFORE you lay on the surgical table and I am not saying that this is

not a forum to ask these questions, I just seem to think that some of

the wording does not appear to be in an attempt to make a surgical

decision, but used a some sort of scare tactic.

Like I said... for me it's not working. I'm doing the dance October

25th! YEAH!
