There are still a few (many) insurances that ask for it. Usually one or two sessions with a licensed psychologist or psychiatrist (they may not accept a counselor and may be looking for a certain licensure) is all it takes. You do not need formal testing. It is to evaluate your fitness and readiness for this type of surgery and the major life changes to follow. Are you informed, do you understand, do you have support, can you handle stress.....etc. Then a letter from that Dr. to your insurance indicating that you are cleared should suffice. I hope he replies, but I would think your Dr's letter should be just what they are looking for. I hope you get reimbursed. Why self-pay even if you can.
