RE: Vest problems

We do often see that when a patient re-starts their Vest treatments, they bring

up a bunch of stuff that has been sitting there for a long time. If she is

coughing so violently that she passes out, that is not good. Not only that, but

she will come to hate her Vest treatments and not do them.

She needs to start out slow...have her do 7 -8 minutes at 10 Hz with a pressure

setting of 4 (1 if it is the Chest Vest) twice a day for now. This way her

lungs can get used to it again. Then she can slowly start increasing the time

and the pressure and the frequency (Hz) until she has reached the settings the

MD ordered.

She needs to learn how to huff cough instead of glottal coughing (what she is

doing now and what you and I do when we cough). Huff cough is much more gentle

and will prevent the bronchospasms that she is experiencing. That may be what

is making her feel faint. The best way to learn how to do a huff cough is to

get a toilet paper cardboard roll, put your lips around one end and blow the air

out forcefully and continuously. That is a huff cough. It is just like when

you blow into the PFT machine. There will be an urge to glottal cough, but she

needs to resist that.

Encourage her to visit her MD and get the RT in the office to review her Vest

treatment with her. If the office doesn't have an RT, just have the MD write a

prescription for Vest retraining and send it to us. We'll help her get

re-started.
