Since I don't have high blood pressure or diabetes by records are pretty much normal... except my problems with GERD, ulcers, and postpartum depression. ( of course I haven't been to the same doctor more than a few times in the past 5 years!! ( we have moved alot) so that should make finding records FUN!! Would he need any of that?? Thanks! Guest guest Posted September 20, 2000 Report Share Posted September 20, 2000 Cindy, I am scheduled for surgery on sept 25th. I was in a similar boat as you. All I had was my discharge summary and H & P from 2 C-sections and a discharge summary for a sinus surgery because it was done under general anethesia. I did include my gastroenterologist report on my GERD. That is all the past medical history I had and I was approved. My PCP did do a detailed H & P though. Hope this helps. I don't think that I enclosed any additonal medical records except a recent c-pap testing report.
