The check-in form is on the www.clos website in the left-hand column

on the first page under " Follow-Up. "

The form asks for our cholesterol and triglyceride level. Do you

know if we need to have this checked before submitting the form?

Hope this info helps. I've been following your story and am sorry

you've been having some problems with food, etc. How's the weight

loss? I'm going to run into my doctor's office on Friday to get my

" official " one-month loss.
