question about surgery dates

I am still waiting on my pcp to get my information ready for me to

pick up....seems like it's taking forever....I have everything else

ready to go. I already have my insurance approval.

Does anyone know how booked up Dr. R is right now and how long does

it take to hear if he approves you or not? Hopefully I can send my

packet in the first of next week via overnight mail.

I am so nervous that something will be wrong with my packet...I've

been working on in it for months....wrote my letter and revised it

numerous times...
