Terri,it's just a recommendation that you fill out the sentence summary part of

the consent form ahead of time. They may let you sign that one? But even if they

don't,it would be a nice guideline. While I was busy " preparing for my

battle " (LOL), I found it tedious-albeit necessary. I said " Braveheart didn't

have to do this " , as my hubby laughed and rolled his eyes.
