Dr. R.'s check-in form - question for post-ops

Hi:

Could some kind post-op soul forward a copy of Dr. R.'s check-in form to me?

I received a copy by email the night before my first (cancelled) surgery,

promptly lost it, and now, as I approach my first month check-in, would like

to find a copy if possible.

If it's lurking somewhere on the clos.net page, let me know - or if you have
