I've had 5 LPs and 4 of them were outpatient, one was in the hospital.  I needed a patch for all of the outpatient ones, and the only reason I didn't need one for hte inpatient was because they pre-emptively gave me IVs of caffeine for 24 hours which made me so jittery that might've been worse.  That said, when I went in for the 4th patch, I had a great anesthesiologist who talked to me the whole time. He told me that some people are just prone to needing them, while others aren't.  It won't matter how hard you try, your dural space won't fill back up with fluid on its own.  I'd go in and get it.  If your ER is like most, then you'll need to express great pain for them to really do it, but there's no reason they shouldn't.  That's my 2 cents...Good luck, and I hope you feel better!  Having had 4 of those headaches, I can feel your pain, and it is AWFUL!!!

I had a LB on Wednesday afternoon and while the procedure went exceptionally well I started to get really bad headaches and nausea about 24 h after, and TODAY my work sent me home in a taxi after I almost vomited in the bosses paper waste bin...so embarrassing! I've tried all advised stuff like neurofen plus, coffee, lots of fluids and bed rest but every time I try to stand-up, the headache intensifies and I just want to hurl. I'm just wondering how soon after a LB a blood patch is warranted? It's been 48 h so far...

I had 5 LB's and each time the dr told me if the headache lasts more than 24hours to call right back to come back and have the blood patch right away. Your leaking spinal fluid do not let it go any longer ...give them a call..

What were the LB's to treat? I just turned my doctor down for one...

they were not treating my NS they were checking the levels in my spinal fluid . The first 3 were to show i had inflamation. it shows up in your spinal fluid. than the other 2 were to see if the meds were working and see if the inflamation was still high.

the LB is a good tool for NS..

What were the LB's to treat? I just turned my doctor down for one...

Ok. Do they hurt? My doc wanted to do it for back pain. My sarc doctor up and moved on me and cancelled all his appointments and left me hanging. no they do not hurt just pressure and uncomfortable if you get a dr who can't do them well.

oh i bet that is upsetting you lose your doc. was he your neuro or lung doc or rheumy?

Dear Sherrie, I think that this happens way to often, but to answet your question. I sounds like you have all the symptoms of needing the blood patch.. everything I've read says 2-3 days.. so its time!! I'd go today while he's in the office and not on holiday.. Saturday, Sunday and Monday and have to go to the ER.. Give your doctor a call today.. I'd call right now..

Sherrie I sure hope you've gotten this taken care today.. I'm sure you had to go to the ER, and well considering what's going on, its one of the things that you "HAVE" to do.. I've had gosh 9-10 done myself over the last 9 years, and they have to do mine under fluroscope and I've been outpatient everytime.. I've never had to had the blood patch before, but its somthing its good you know about..... I can remember once getting a slight headache, I just stayed in the bed with my head flat, and it always went away.. but if its disturbing your balance you vision and you're wanting to hurl everytime you try to look at someone, its time to go to the er..I hope your feeling better by tonight..

I see I'm way late on my response, had Remicade this week and still a little slow. LB are inconclusive for diagnostic testing for Sarcoid, usually.  It may or may not show inflammation in the body.  I had one, inconclusive, and my neuro said no more, my

symptoms again were enough.  On the Blood patch I hope you have already called your doctor, if not please do, you will need the blood patch to get over this, it is well worth it. Dianne, on turning the LB down, I think your doctor (if I remember right) is doing it for diagnostic reasons??  I'm sure we have all had a least one for this reason if not more, I also know that many times we are told it was normal, that doesn't mean a thing when it comes to NS, if your doc is trying to rule out other diseases, then it might be worth it. 

I just know for me, I went through hell, before saying enough, please just treat me!!  My doctor finally listened:)  So there are times when we do need to say hey stop all the  madness and do something to help me here.

I've only had one LP, but I've had spinal anesthetic twice. No problems with first spinal (for c/section), 2nd spinal I got the headache & got the blood patch the 2nd day. One of medicine's miracles! Even though I'd seen it many times as a nurse/midwife, to experience it is amazing. From horrible headache to total relief in 5 minutes--incredible. The only LP I had was done in the neuro's office, no fluoro or anything, just his experienced hands (I'd worked with him many years & had a lot of confidence in him). Had no problems at all. I will say that no matter what the discharge instructions are regarding activity, etc., I'd stay down for 24 hrs. Drink the fluids & caffeine, but stay down. With the spinal for anesthesia (outpatient procedure, not the c/s), they told me I could be up after 4-6 hrs., so I was. Ohhhhhhhhhh, big mistake. So when I had the LP, I drove straight home & down I went & down I stayed.Ramblin' RoseModerator A merry heart is good medicine. Okay, my OCD is kicking in here. I don't know if nearly everybody is making the same typo, but I want to make sure the new kids especially realize that it's LP (for lumbar puncture), not LB (which are my dog's initials, for Little Bit, as well as my neuro, Larry Blankenship). There, now Mr. Monk & I are both happy. Dang, I'm going to miss that show.

Hey Rosie, I've had almost a dozen.. the first time he tried to stick me 5 times without success..It was my first visit with the Neuro/Immuno, I don't know if I trusted him or not, but I didn't trust him then and I was smart for not, he was the one with the big bushy eyebrows, he is from India, and I didn't understand a word he said the first time, and with my strong southern brough, he didn't understand me, we both kept asking each other to repeat the other one, I'd say "Sir" and he kept saying "excuse me?" it was a horrible time in my life and he was gonna damn well prove me wrong about NS that I didn't have it.. Trouble was my ACE was over 200.. with my Sed rate close behind.. so to me if it wasn't Sarc, then obviously I had something extremely wrong.. I would have gladly given up the dx of NS..but every peice of lab work was saying yes.. I'll shut up now.. I've never had a blood patch but due to my body size, and my abnormal spinal make up someone had to do this by floroscopy..

Oh yeah, , I know that some people need to have the fluoroscopy, but I don't think we all need it. Lots of docs use body size to justify their inability to do an SP, spinal, epidural, etc. When I worked at the University Hospital in San , the anesthesiology residents were actually taught clinical skills by CRNAs (certified registered nurse anesthetists). The docs were always asking for extra long needles to do labor epidurals, blaming the patient's size if they couldn't get the epidural in, sticking them multiple times. One time we had a very large lady & the resident tried several times. The nurse went to get the CRNA, who told him that he didn't need the big long needle. She positioned the lady, felt her back very carefully, used the standard needle, and got the epidural in on the first attempt. The doc was flabbergasted. I just hope he always remembers this incident, that skillful hands are one of the most important factors. Doesn't mean that you don't need the technology, but I don't believe the technology replaces clinical skill. Hi all!

Thanks so much for the support! I held off until Sunday morning and didn't have

the procedure yesterday afternoon (it's now Tues morn). I'm currently sitting

up in bed and feeling SO MUCH better!! The nurses in my ward (I'm a `border' in

the gastro ward because there was no room elsewhere) are in a little bit of

shock because I was on my back with sheets over my head and not so talkative all

that time prior to the op.... How amazing that something so little can make so

much difference!!! Hopefully the LB was worth all the trouble (:
