I've seen some documentation on the internet saying that certain places will approve 12mm and up. Because CT was out of network, we had to pay up front, and CT filed all of the paperwork for us, and I just received a check about a week ago. I discovered that Oxford actually has a " clinical guideline " document posted on their website about plagio and bands as well. Apparently their requirement is asymmetry >12mm, which fortunately or unfortunately we met without problem - Ellis was 17mm. In fact, they covered band #2 without any issue as well. We feel very lucky since i've seen all the posts from others about the challenges they face with their insurance! Guest guest Posted January 20, 2010 Report Share Posted January 20, 2010 We have been very fortunate, also. We have Aetna and they have covered our helmet 100%. In fact our first appointment at Hanger was on Dec. 17 and by Dec. 24 we already had our helmet and Aetna has already sent the check in. I think they require >8 mm and some physical therapy first.
