It's really what you're comfortable with.  They generally recommend banding with a difference of 10mm or more for plagio.  Some have chosen to band at 7mm, others don't.  Repositioning does work, but the more mobile your baby gets the less effective it is, especially with tort in the mix.  When they are sleeping they will move to whatever they find most comfortable (usually on the flat spot) and there's isn't a lot you can do to prevent it after a point.

As for when, the earlier the better.  You'll catch more growth and see more improvement faster, meaning the time in the helmet is less.  If you get evaluated and decide to band, don't wait.

His doctor noticed a flat spot on the back right side of his head and referred us to pediatric physical therapy. There we learned there was an 8 mm difference in measurments, and that he has torticollis. We were taught stretches and positional therapy.
