Guest guest Posted May 9, 2006 Report Share Posted May 9, 2006 When the foot lifts up his toes are in the air and it does roll to the outside edge of his foot. He just started standing a week ago so I was not sure if this was normal since his other foot does it a little too but not as much as the clubfoot. I am concerned and have that nagging feeling that something is wrong. The doctor said that the foot looked really good and was very flexible etc....but now I am nervous. We are located in Rochester, NY. As far as I know the closest Ponseti certified doctors are in NYC about 6 hours away. I'd pursue it, to give you peace of mind. The docs in NYC are supposed to be very good. (Hopefully others in that area will post to you too.) In the meantime you could also post the pics to the CFPics group here (are you a member?) for feedback here. Take pics of him standing/bearing weight, from the back, the front, the side, maybe also holding the foot showing the dorsiflexion (the amount the foot can lift up flat from the heel). Guest guest Posted May 9, 2006 Report Share Posted May 9, 2006 So Stacey, Are you saying that his foot only does this when he lifts it? Or is he actually rolling out onto the outside edge of the foot when he is standing with weight on it? Everything that said is right on, relapses should not be messed with, and re-casting to correct is definitely the first step. Many relapses can be corrected by casting and fewer ATTT's (anterior tibial tendon transfer, which I'm assuming is what he is talking about) are needing to be done because of new casting success with older kids. Essentially what the ATTT does is stabilize a foot that has been continuing to try to relapse - but the first step, again, should definitely be repeat casting. Based on your description of the foot rolling to the outside edge, I'm just not sure if it's a relapse or not. Especially if you are seeing it with his other foot some as well. When my son sits (he's 19 months and unilateral) and relaxes his feet, both feet have a tendency to sort of curl up on their sides -- I've been told that this is perfectly normal. You said his foot is still very flexible, has he lost any of his outward rotation? I think the key thing is whether or not he is standing on the outer edge of the foot when he is putting weight on it. But again, agreeing with you would do well to email photos to Dr. Ponseti, and of course if you want to get our " un-expert " opinions, feel free to send some pics on over to CFPics. Guest guest Posted May 9, 2006 Report Share Posted May 9, 2006 Sometimes his outward rotation looks the same to me and other times it looks like he has lost some of it. I think I look at his feet too much and I do not know what I am seeing anymore. How do I post pictures onto the site? I could just email pictures to Dr. Ponseti and he will respond? What address do I send those to? When stands his legs are pretty far apart still. So, maybe there is not as much weight on that foot as I think. When he does sit both feet sometimes also look like they curl up a little to the side. The doctor did say this was normal but the rolling out on his foot he made it sound like it wasn't and there was nothing to do about it now. But, he actually did not have stand up for him to see now that I think about it. Guest guest Posted May 9, 2006 Report Share Posted May 9, 2006 Our daughter just started standing up a month or so ago & initially I did catch her occassionally standing on the outside of her cf. Scared me because I know it can be a sign of relapse. But she had just had a great checkup so I decided to wait a week or two & see what happened. Seems to have just been because that leg/foot/ankle were a little weaker than the other non-cf side & she was hesitant to bear weight, I think. Now that it's a few weeks later, her cf turns out (over- correction) when she bears weight/stands, just like it's supposed to do. I definitely would have had her back to the doc if she continued to stand on the outside of that foot. Keep an eye on things & follow your most conservative instinct. Here's my thoughts, you mentioned that your doc is one of the best so I'm not overly worried for you (assuming that you got that advice from us that is : ) ). If it is that he is relapsing I would think that any of the top Ponseti docs would have recognized this and wanted to recast. If it is a muscle imbalance that may or may not require the ATTT later, this IS something that can resolve on it's own, some kids have been known to thhave growth spurts in which that tendon tightens up, but then after the growth spurt the tendon sort of has a chance to " catch up " and balance the foot out again. Also, since he is just starting to be weight bearing you may see this stop after he has been on his feet a while. The pics can be posted via email attachments to the CFPics group. In it you'll find a short survey to fill out and return, then your membership can then be approved. In this situation, if you have already talked to the doctor about it the next best step I can recommend is sending the pics to Dr. Ponseti if you feel you need a second opinion. And yes, you can just send him an email with the photos and ask him whatever you want to know. Use the positions that mentioned for taking the pics. Actually, I have heard that he is a great doctor from many members of the medical community around here but not heard any feedback about him from this site. We always thought that 's foot looked fully corrected. He was evaluated by early intervention when he was 4 months old and they also said he looked good. I called them today and hopefully should hear back from them because they mentioned he could be re-evaluated when he is 10 months old. (which is now) I should take pictures of 's feet while he is standing from the front, back and what else? Forgive me, my mind is so full of information lately.
