We are in the s. My daughter too grows out of the shoes in width wise

before length. J. will make a wide width for your little one if that is

what they need. This is the beauty of them! Besides the comfort and less

problems. They actually send you an impression kit, and take an impression of

the foot so that it fits good. If he grows out of them width wise and it gets

too much my daughter actually got pressure sores on the outer side of her foot.

So watch for that. As for billing insurance I have been told that if your

Orthotist writes them as a 'Ponseti/ shoe and bar AFO' <----thats the

key word. Then they are more willing to pay for them. But make sure the

Orthotist doesnt rip off the insurance Co. or they may not. I have heard alot of

Orthotists will charge the insurance Co. like $2000 for them and they only cost

$350 for the shoes and bar or $296 for the shoes. Good Luck!

Hi Donna,

I would think that if the shoe is too narrow, you would not be able

to place her foot flat in the shoe. THat would pose a problem in

that if she isn't flat in them, then she may be settling on the

outside of her foot, thus possibly opening herself up to relapse. I

would definitely inquire about wider Markell shoes. You also don't

want to get the shoes too big because that causes its own set of

problems.

The difference with the s is that takes an

imprint of each child's feet to make the shoes. Therefore they come

wide enough. Also, it's been my experience that he makes the shoes

with room to grow. I have only had to buy 2 pairs of

shoes. Whereas, I would have bought multiple pairs of Markells by

now.

I don't know what your insurance is like, but my insurance will only

pay for the bar, not the shoes, so I was paying something like

$65/Markell shoe anyway. Yes the s cost more and insurance

is tricky, but a phone call is inexpensive if you want to get

questions asked.

HTH,

RCF (3/30/04) 2 years old on Thursday!!

Definitely ask about the wider Markells, it doesn't make sense that

a company who deals with wide feet doesn't make wide shoes.

Donna,

have you seen the flyer from Markell on the shoes? The standard boots

come in medium and narrow (my daughter has always had narrow) but the

newer style with the opening to view the heel comes in Narrow, Medium

and Wide.. scroll down to see those:

http://www.markellshoe.com/ponsetiflyer.pdf

hope this helps,

NO, they have never put her in the wide width Markell's. They have

always given her a bigger size to accomodate for her width, but now

she is in a 2, which does fit lengthwise, and I looked online and

they don't make the bigger Markell's in wide. I am completely

baffled because I thought most clubfeet were wide!!

You are right, that is exactly the problem. Her feet cannot sit flat

in the bottom of the shoe. They seem to be bowing up in the middle,

and then she gets these red marks where the strap presses against

that area. Ridiculous. We go tomorrow to deal with this problem. I

would like to consider the 's, but what do I do with her in

the meantime? She is almost 17 months old and I would hate to put

her in holding casts when she is just learning to walk! But I don't

think I can stand putting her in the Markell's for one day longer

than I have to. She absolutely screams when I put them on, but I am

afraid not to. She settles once they are on, but I know they don't

fit so I feel really guilty.

Thanks for your reply. I don't understand why they don't offer wide

in larger sizes. It isn't like a 2 is a very big shoe!! I did send

them an email but haven't received a reply yet. It is getting very

frustrating. I was hoping that Jay would see my post and reply, but

no bites!

Christee,

I go to the ortho fitter tomorrow, and then to the doc. I am

seriously considering telling the ortho guy that I want the

s and directing him to the web site. I know that he has

never used them. I may look into if insurance will pay out if we

order them direct because I do worry about the " mark-up " of the

price. I will have to be upfront to the ortho guy about the price

and all, since I do know what they cost. Although I don't have the

resources to order them direct, has a doting grandma who

would probably let us borrow the amount. So you have used both,

right? What have been the downside, if any, to the 's? How

long should i expect to wait to receive them once they receive the

impression kit? I drive an hour and a half to the ortho's. Since

they don't have an impression kit, and I would have to order one, do

you think that I could get it at my home, do the impression, send it

on to and still have the shoes billed direct to my insurance

through the ortho fitter? Do it all myself but have the ortho guy do

the billing. Does that make sense?

Thanks for your input.

Well that's just silly. Why in the heck doesn't Markell make the

wider widths in bigger sizes? Is that new? I swear they came in

wides before but I can't find my old copy of the flyer. I would be

contacting Markell to try to find out. Are you out there Jay? What

is up with this??? Personally I think we should start a letter writing

campaign to Markell to encourage them to make the changes we've been

suggesting for ages.

Donna,

It may sound crazy, but what about putting her in a pair of regular

shoes that fit, provided you've been able to find some : ) and duct

tape the shoes to the bar every night? Certainly not an ideal

solution but it could work while you wait for s.

Donna,

I think you can use regular florist foam in a pinch if you can't wait

for the impression kit. I think people have done that in the past but

I don't know if will still let you do it that way.

ummmm... you can't take them off the bar like the markells on the

gold bar. and... well... not a whole heck of a lot I would have to

say. They're not perfect yet, but I am sure they will be soon. Then

again... nothing's perfect right? I think he's done pretty good for

only having made them for about 2y now. At least he's willing to

work with any problems that do come up and has made numerous

changes/improvements in that time with the feedback he gets.

Darbi wore Markells for almost 2y. She had problems at first, but as

many of you know her trauma laid the groundwork for the tips document

we use now so that's ok... she was fine in her shoes really though

and I have always said they are a good shoe for most CF

children. Most... So our switch was due to other reasons, namely

that I felt the toe box in the Markells was compressing her toes too

much and making them curve. The PM's have a much roomier toe box and

I wanted that for her if she was to have any hopes of not having toes

that looked like C's. So far, I'm one of a very few who has had this

issue but for us, the switch couldn't have been better. I really

like the design of the PM's and many things are better now (even if

they weren't BAD before... they are better). She has never once

asked me to massage her legs since she got them, I can see that her

toes aren't getting any worse (even if they're not getting any

better...), she can walk unaided flat footed even on hardwoods. All

in all... very good.

>How

>long should i expect to wait to receive them once they receive the

>impression kit?

about 2w. Or less. They can be expedited, just ask to get them

to you as soon as he can and tell him why. They usually overnight

them anyway.

> I drive an hour and a half to the ortho's. Since

Pretty sure, yes. If they order them for you, just call and ask

him to send the kit to your home instead of the orthotist's shop. He

is pretty good about that kind of thing.

I think this may be your only option now. I didn't know the larger

sizes didn't come in wide either. That's about stupid. So you've

probably got no choice I guess. Have you checked with insurance to

see if they'd pay? There are codes you can give them so they can

check. They were posted here about 3w ago I think actually. If I

can find the post I will send it to you so you have the codes.

Well, I went to the ortho fitter yesterday and we talked about the

's. He is aware that they are out there, but said that my

insurance would not cover them. Ironically enough, he told me that

he just met Dr. Ponseti last week. I think he said there was a

conference somewhere. Anyway, I told him about the codes used for

the 's and he said that since he would have to justify the

codes, he could not use them for the 's because they are not

actually AFO's. It was so refreshing to talk to a completely honest

provider, even though he wouldn't help me. I think it is ridiculous

that the would have to be called AFO's in order for insurance to

cover them. Would insurance rather pay for repeated castings when

our babies relapse if they did not have the shoes? Anyway, we got

another pair of Markell's. He thought the other pair fit her ok but

I could barely get them on her because of the volume of her foot.

I left them on her for the 15 minute drive to her ortho doc, and by

the time we got there she had hot spots on her heels. So he sent us

back to the fitter to put gel pads in the shoes, which she had in

the previous 2 pairs, but he refuses to put them in automatically.

He insists on seeing if blisters are likely to develop first. He

doesn't seem to care that I am driving an hour and a half to get to

him and don't want to go all the way home, only to have blisters

develop and have to turn around and come back.

He commented on the length of the bar since we had lengthened it.

The shoes were about 4 inches apart heel to heel and she was so

uncomfortable that we lengthened them to shoulder length. He

measures bolt to bolt and he was annoyed that we had messed with

them. He was sure glad to see us leave! Her doctor said we are doing

great by her so I don't care if the fitter disagrees with me about

the bar length or anything for that matter. Her doc only prescribes

the FAB until the kids are 3, and I am planning on keeping in

it until at least age 4 and maybe age 5, so I will still have a

chance to switch to the 's if it becomes necessary.

I took 's " foot book " with me to show her doc since he didn't

meet her until she was 4.5 months old. He and his assistant were

surprised that her feet were so severe at birth, since when he saw

her she was almost corrected to neutral. I made a scrapbook of her

foot journey with lots of pictures and information about clubfeet

and treatment. I hope that when she gets older she will really

appreciate knowing without a doubt what she went through. Anyway,

the doctor says " let me show you my book " and he pulls out a medical

journal that was just published that he was the editor of. There is

an article in it about clubfeet by Dr. Morcuende (I'm not sure if I

spelled that right). Another article is by my daugther's doc. It was

pretty cool. Makes me feel good to know that her doc is current and

involved in the field. It was so cute, you could just see how proud

he was of the publication!! Probably the same way I look when I show

off 's book! No one saw her feet because she was casted before

we came home to the hospital, so I really like for people to see

where she started.

Anyway, thanks for all of the help. I hope that we can avoid

blisters and make these Markell's work, at least until next year

when we could use our Flex Benefits to help pay for the 's.

I think part of her problem with the shoes is that I lay her down to

put them on and she has had an ear infection since January! She goes

in a couple of weeks to get tubes, but meanwhile she can be really

miserable at night!

Just thought I'd mention that they are now called AFO's, actually it's called a

Ponseti AFO. We just got them a month or two ago and our insurance paid for them

completely. Good luck and sounds like you have a good orthotist, maybe he could

look into this as it is a recent change - both in coding and design. Now the AFO

comes up the back of the ankle/calf area a bit more, giving it more stability

and coverage and also qualifying it as an AFO.

Re: Width of Markell's?

Hey Donna,

A couple things, first, did they actually find a new size that will

fit her comfortably?

Second, the whole reason that the PM's can be called AFO's for

insurance is that changed the design specifically to meet the

insurance guidelines for what constitutes an AFO - therefore any

orthotics shop CAN get them and bill your insurance using the AFO's

code. You can also get them directly from and submit it to your

insurance company yourself using those codes.
