Dr. Macura also has the post-ops filling out a simliar questionaire.

***It's true. We have to fill out a questionnaire everytime we go in

for an appointment. It asks about how much sugar we've had, protein,

dairy products, etc. Then it also asks how much exercise, how far can

we walk, how many flights of stairs we can go without stopping (I can

do three now!) and whatever other exercise we are doing. Also asks

about any symptoms we might be having like gas, diarrhea, reflux and

nausea.
