It was suggested by several well-seasoned members of this group to put together a documented file of your child's medical history. I have two sets of these files - one for myself - and one I plan to give Dr. Harbison. What she chooses to do with the information is up to her. But, at least I will be prepared! I did Ian's as follows: 2 binders with 6 sections - each with metal tabs at the top to hold the paper in place. Each section labeled with the appropriate documentation as follows: 1st Binder 1. Birth/Neonatal 2. Surgeries 3. Nutrition 4. Speech 5. Hospital 6. Pediatric 2nd Binder 1. Endocrinology 2. Gastroenterology 3. Genetics 4. Growth Charts 5. Laboratory 6. Radiology Each section has it's own respective medical documentation. I make sure that each time I see the doc - I put the documentation immediately where it belongs. I just started doing this - and BOY, has it been a GOD send! It is so much easier to keep track of all Ian's medical records - and helps me keep track of what's next! I also have a separate white folder containing pictures of Ian that I place inside the first binder. I also printed for myself and Dr. H a list of all the contacts (Ped, Endo, Gastro, Genet, and Hospital)with names of physician, practice, address and phone number so that she will be able to have that information handy if needed.
