HELP! Any Mt Sinai patients with a scanner?

Hi folks!

Tomorrow I have my consultation with Mt. Sinai's

nutritionist... I'm doing it over the phone because I

live south of Philly, and Mt. Sinai is in NYC.

Anyway, the nutritionist asked me to fill out some

form and fax it over to her-- but I don't have the

form! Blargh! Supposedly this form came in the

original packet o' info they sent... but I don't see

it in my packet.

Do any of you Mt. Sinai people know what form they're

talking about, and, if so, could you possibly scan it

and email it to me??

I know I should be able to call them in the morning

and get a copy, but they're very hard to get a hold

of.
