My son also probably has one of the more extreme cases of insulin resistance, as he isn't doing very well with his growth even on GH. What I have learned is that as long as the bone age doesn't advance, you don't really need to treat the other adrenarche issues. Even if your son is put on an aromatase inhibitor, it will not stop the overt signs of adrenarche such as the body odor, acne, etc. Your doctor should do bloodwork to look at DHEA levels, and if there is any question about those levels, an ACTH stimulation test can be done as well. I have not heard of the risk of tumors associated with elevated IGF-1 levels, but there is a risk of Type 2 diabetes, as we are dealing with this issue now.
